Who might be able to join this trial:

• You have not previously received any systemic (whole-body) treatment for extensive stage small cell lung cancer (a more advanced form of lung cancer that has spread widely)
• You have been diagnosed with extensive stage small cell lung cancer through a tissue/pathology test, and your doctors have determined you cannot tolerate receiving radiation and chemotherapy at the same time
• You previously had an earlier, less advanced stage of small cell lung cancer and were treated with radiation and chemotherapy, but your cancer came back at least 6 months after that treatment ended
• If you previously had radiation to a tumor, that tumor must show clear new growth, and there must be at least one other measurable tumor in your body (confirm with trial site)
• You are 18 years old or older
• You are able to carry out light activity and care for yourself (your doctor will assess this using a standard scale called ECOG, scored 0 or 1)
• Your doctors expect you to live for more than 3 months
• Your blood counts, liver function, kidney function, and heart function all meet specific healthy levels — your doctor will check these with blood tests and a heart scan
• If you are a woman who could become pregnant, you must agree to use contraception during the study and for 6 months after it ends, have a negative pregnancy test within 28 days before joining, and not be breastfeeding; if you are a man, you must also agree to use contraception during the study and for 6 months after it ends
• You are willing to sign a consent form and are able to follow the study requirements reliably

Who may not be able to join:

• You have brain metastases (cancer spread to the brain) that are causing symptoms, swelling in the brain, or that require radiation treatment
• You have an active autoimmune disease (where your immune system attacks your own body) that has required ongoing medical treatment within the past 2 years
• You have been diagnosed with an immune system deficiency, or you are currently taking steroid medications or other immune-suppressing drugs at significant doses within the 2 weeks before joining
• You have had another type of cancer within the past 5 years that has not been fully cured
• You have difficulty swallowing, have had part of your digestive system surgically removed, have ongoing severe diarrhea, or have a bowel blockage that would make taking oral medication difficult
• You have fluid build-up around your lungs, heart, or abdomen that cannot be controlled and requires repeated draining
• You have pressure on your spinal cord that cannot be treated, or had spinal cord compression that has not been stable for at least 1 week before joining
• You had significant coughing up of blood within the 2 weeks before joining
• You are still experiencing side effects from a previous treatment that have not returned to a mild level (except for hair loss)
• You had major surgery or a serious injury within 28 days before joining
• You experienced a serious blood clot event — such as a stroke, deep vein clot, or pulmonary embolism — within the 6 months before joining
• You have a history of psychiatric drug abuse that you are unable to stop, or you have a diagnosed mental health disorder
• You have poorly controlled high blood pressure, significant heart problems (such as heart attack history, certain irregular heartbeats, or heart failure), a serious active infection, liver disease or active hepatitis B or C, HIV, high levels of protein in your urine, or certain other serious health conditions (confirm with trial site for full details)
• You received a blood transfusion, albumin, or certain blood-stimulating medications within 14 days before your first dose in this study
• Your doctor believes you have other health conditions that could put you at risk or prevent you from completing the study
