Who might be able to join this trial:
- Adults between 18 and 65 years old.
- People who have been diagnosed with obsessive-compulsive disorder (OCD) based on standard medical guidelines (DSM-5) — previous treatments do not need to have failed for you to be eligible.
- People whose OCD symptoms are at least mild to moderate in severity, as measured by a standard OCD assessment tool called the Y-BOCS (a score of 14 or higher).
Who may not be able to join:
- People who have been diagnosed with schizophrenia, bipolar disorder, a brain-related psychotic disorder, alcohol use disorder, or drug use disorder.
- People who have been assessed as being at high risk of suicide within the past year.
- People who currently have a serious or active medical or surgical health condition.
- People who have had, or are planning to have, brain surgery, or who have any metal implants in their brain or body (such as a brain stimulator or heart pacemaker).
- People who have abnormal structures in the brain (such as a brain tumour) or neurological conditions (such as epilepsy, stroke, meningitis, or encephalitis).
- People who are pregnant.
- People who are unable to tolerate being inside a scanning machine (PET/MRI) due to claustrophobia or severe anxiety in confined spaces.
- People who, for any reason, may not be able to follow the study requirements — including those unable to cooperate, unable to give informed consent, or deemed ineligible by the research team after an initial screening.
- People taking certain medications (such as some antidepressants or antipsychotics) that may increase the risk of seizures or suicide.
- People with a known allergy to a substance called 18F-FDG, which is used in the brain scan.
- People who have previously had a bad reaction to TMS (a type of brain stimulation) or are allergic to the positioning cap used during TMS.
- People who have any metal objects within approximately 30 cm of their head (in the area where the stimulation device would be placed).
- People who have previously received, or are currently receiving, treatment with electroconvulsive therapy (ECT) or vagus nerve stimulation (VNS).
