Who might be able to join this trial:

At the start of the trial:

• You are 18 years of age or older
• You have kidney cancer (renal cell carcinoma) that has spread or cannot be removed or cured by surgery or radiation
• Your kidney cancer has been confirmed by a lab test and is mainly the "clear cell" type
• Your cancer is rated as "favorable" or "intermediate" risk based on a standard medical scoring system called IMDC
• You are reasonably active and able to carry out daily activities, as measured by a standard health scale (Karnofsky score of 70% or above)
• At least one area of cancer can be measured on a CT scan
• At least one area of cancer shows up clearly on a special type of imaging scan that looks for a protein called CD70
• If any cancer areas do NOT show up on the CD70 scan, there are no more than 3 of them, they are not in major organs, and they make up less than 30% of your total cancer (confirm with trial site)
• Your blood counts, liver, and kidney are working well enough as shown by recent lab tests
• You are able to understand and agree to follow the trial's requirements, and have signed a consent form
• If you are able to have children, you agree to use reliable birth control during the trial and for 5 months after your last treatment
• If you are a woman who could become pregnant, you have a negative pregnancy test before starting

At the 12-month check-in point:

• You have met all the starting requirements listed above
• You have completed approximately 12 months of standard first-line treatment (a combination of two types of cancer medicines targeting PD-1/PD-L1 and VEGFR) without having to permanently stop both medicines due to serious side effects
• You have had the special CD70 imaging scan at around the 12-month mark
• Your cancer has either responded well enough to consider pausing treatment, or has not — both groups may continue in the trial under different pathways (confirm with trial site)

Who may not be able to join:

At the start of the trial:

• You have already received treatment for advanced kidney cancer
• Your cancer is rated as "poor" risk based on the IMDC scoring system
• Your general health and activity level fall below the required threshold on the standard health scale
• Your blood counts, liver, or kidneys are not functioning well enough
• You have active cancer that has spread to the brain or the lining of the brain, unless it has been successfully treated and is stable
• You have had another type of invasive cancer within the past 5 years, with some exceptions such as certain skin cancers or early-stage cancers that have been fully treated
• You have serious uncontrolled health conditions, such as uncontrolled high blood pressure, significant heart disease, serious gut problems, significant bleeding, severe active infections (including active HIV, hepatitis B, or hepatitis C), or severe autoimmune diseases
• Your life expectancy is estimated to be less than 6 months
• You have a known allergy to the special CD70 imaging agent used in this trial
• You have severe liver or kidney problems, or cannot tolerate the PET/CT scan procedure
• You have medical, mental health, or social circumstances that would make it difficult to follow the trial's requirements
• You are pregnant, breastfeeding, or unwilling to use contraception during and for 5 months after treatment

At the 12-month check-in point:

• You were unable to complete approximately 12 months of first-line treatment because of serious side effects or because your cancer got worse
• You were unable to have the CD70 imaging scan at the 12-month point
• Your health has declined significantly, or your doctor does not feel it is safe or appropriate for you to pause treatment
