Who might be able to join this trial:
- People aged 75 or younger at the time of joining the trial
- People who have been diagnosed with prostate cancer (specifically a type called adenocarcinoma) confirmed by a tissue sample, who have not yet received any treatment for it
- People whose overall health and mental state allow them to follow the study's requirements and attend follow-up visits
- People whose cancer has reached a certain stage — specifically, the tumor has grown beyond the prostate capsule (T3 stage), or in some cases has grown into nearby tissue but is still considered treatable (T4 stage), as determined by physical examination or MRI or other approved imaging

Who may not be able to join:
- People with a PSA blood test result above 100 ng/mL
- People with any other medical condition that, in the opinion of the trial doctor, could interfere with the study results
- People with medical reasons that make both surgery and radiation treatment unsuitable for them
- People with certain bowel conditions such as anal or rectal fistulae, Crohn's disease, or ulcerative colitis (which may affect suitability for radiation)
- People with significant urinary blockage or obstruction symptoms (which may affect suitability for radiation)
- People with a narrowing of the upper part of the urethra (which may affect suitability for radiation)
- People with severe nerve-related bladder problems (which may affect suitability for radiation)
- People with a very enlarged prostate (generally beyond 70–90 mL) (which may affect suitability for radiation)
- People who have previously had radiation treatment to the pelvic area (which may affect suitability for radiation)
- People where the tumor has spread very extensively in the local area, particularly toward the base of the prostate (which may affect suitability for surgery)
- People with severe abdominal obesity (which may affect suitability for surgery)
- People who have medical reasons that make general anaesthesia unsafe (which may affect suitability for surgery)
