Who might be able to join this trial:

• You have been diagnosed with invasive breast cancer (confirmed by a tissue sample) within the last 5 years
• Your cancer meets at least one higher-risk category: cancer that has spread to underarm lymph nodes; "triple negative" breast cancer; certain hormone-positive, HER2-negative, lymph node-negative cancers with a high-risk score on a specific genetic test (Oncotype DX score of 25 or higher and/or a high-risk MammaPrint result); or cancer that was still present in the breast after chemotherapy given before surgery
• You have finished all main treatments (surgery, chemotherapy, radiation, and/or HER2-targeted therapy) at least 4 weeks before joining; ongoing hormone therapy and bone-strengthening medications are generally allowed, though tamoxifen use has specific conditions (confirm with trial site)
• There is no sign that your cancer has come back, based on physical exam, blood tests, and any needed scans
• Your blood counts are at acceptable levels (enough white blood cells, platelets, and hemoglobin)
• Your liver is functioning at an acceptable level based on blood tests
• Your blood clotting tests are within an acceptable range
• If you take blood thinners, your levels are stable and within an acceptable range, and the medication can be temporarily paused for a bone marrow procedure if needed
• Your kidneys are functioning at an acceptable level based on a blood test
• You are willing to have bone marrow samples and blood samples taken as the study requires
• You are 18 years of age or older and able to give your own consent to participate

Who may not be able to join:

• You are currently taking part in another clinical trial that involves an experimental treatment
• You regularly take high doses of steroid medications (such as prednisone or similar drugs) or other medications that suppress your immune system; skin creams or inhaled steroids are generally allowed
• A heart tracing (ECG/EKG) shows an abnormal reading above a certain level (QTc greater than 480 ms)
• You have a serious or uncontrolled heart condition, such as an uncontrolled irregular heartbeat, a heart attack or certain heart procedures within the past 6 months, or moderate-to-severe heart failure
• You have a history of serious lung inflammation or severely reduced lung function (confirm with trial site)
• You have diabetes that is not well controlled
• You have an active or uncontrolled serious infection
• You have significant liver disease such as cirrhosis or chronic active hepatitis
• You have HIV (as reported by you)
• You have a history of major stomach or small intestine surgery, or an ongoing digestive condition such as Crohn's disease, ulcerative disease, or serious problems with nausea, vomiting, diarrhea, or absorbing food
• You have an active bleeding disorder
• You have a history of eye problems affecting the retina or retinal veins
• You are currently pregnant or breastfeeding; women who could become pregnant must have a negative pregnancy test before joining
• You have previously been treated with a CDK4/6 inhibitor drug (confirm with trial site)
• You have a known allergy to hydroxychloroquine or related medications
• You have already taken hydroxychloroquine for more than 1 month since completing your main cancer treatments
• You started bone-strengthening medications less than 3 months before enrolling in the study
• You have a history of Hepatitis B or C — additional blood tests may be required at the screening stage (confirm with trial site)
