Who might be able to join this trial:

• People aged between 20 and 79 years old, both men and women
• People who have been diagnosed with lumbar vertebral facet joint syndrome (a condition causing lower back pain related to small joints in the spine)
• People who have had lower back pain for at least six months, rated 4 or higher on a pain scale
• People who have previously tried conventional pain treatments such as anti-inflammatory medications, opioids, muscle relaxants, oral steroids, physical therapy, or chiropractic therapy
• People whose spine imaging shows arthritis in the facet joints (small joints in the lower back)
• People whose referred pain (pain that spreads from the back) does not go below the knee
• People who have had at least one diagnostic nerve block injection (a small injection of local anaesthetic near the nerves of the back) that reduced their pain by more than 75%

Who may not be able to join:

• People whose MRI, CT scan, or physical examination shows nerve root problems in the lower back, including leg pain caused by a compressed nerve
• People who have muscle weakness or any other condition that doctors have identified as requiring surgery
• People who have had MRI-guided focused ultrasound (MRgFUS) or radiofrequency (RF) treatment for lower back pain in the past six months
• People who have had previous lower back surgery
• People who are pregnant
• People who have an active cancer diagnosis
• People who have known allergies to contrast dyes, local anaesthetics, or sedation medications
• People who have a medical reason they cannot have an MRI scan (confirm with trial site)
• People who have an acute (sudden or short-term) serious illness, such as pneumonia or blood infection, that may prevent them from completing the study
• People with unstable heart conditions, including: unstable chest pain (angina) despite medication; a heart attack within the past 40 days; severe heart failure; uncontrolled or untreated irregular heartbeat or use of medications for heart rhythm problems; or a pacemaker
• People who have had a severe stroke within the past six months
• People with severely high blood pressure (lower blood pressure reading above 100 even while on medication)
• People with an active infection or serious uncontrolled blood, neurological, or other medical conditions
• People who are unable to communicate with the research team
• People who are unable or unwilling to remain still for approximately two hours during the treatment procedure
• People with blood clotting disorders or bleeding conditions, or those who have taken blood-thinning or anti-clotting medications within five days before treatment
• People whose diagnostic nerve block injection did not reduce pain by at least 75%
