Who might be able to join this trial:
- You live within 5 miles or 20 minutes' drive from the emergency department
- You are 18 years old or older
- You are able to give your own consent, or you have someone who can consent on your behalf while you agree to take part
- You can identify a caregiver who is willing to stay with you for the first 24 hours at home, and who is able to contact the medical team if something seems wrong — after 24 hours, they should be available for occasional check-ins (this requirement may be waived for some patients at the doctor's discretion)
- Your main diagnosis is one of the following: a skin infection (cellulitis), heart failure, a complicated urinary tract infection, pneumonia, COPD or asthma, another type of infection, chronic kidney disease, pain related to cancer, diabetes or its complications, a gout flare, very high blood pressure (hypertensive urgency), a previously diagnosed irregular heartbeat (atrial fibrillation) with a fast heart rate, a need for blood-thinning medication, or a condition that requires hospital-level medical care that you wish to manage without procedures
Who may not be able to join:
- You do not have a stable home to stay in
- Your home does not have working heat (October–April), working air conditioning when temperatures exceed 80°F (June–September), or running water
- You are on methadone and need to pick it up daily from a clinic
- You are currently in police custody
- You currently live in a facility that already provides medical care, such as a nursing home
- You have screened positive for domestic violence
- You are currently experiencing sudden confusion or delirium (as assessed by a specific medical test called the Confusion Assessment Method)
- A standard IV line cannot be placed in your arm in the emergency department, or it requires ultrasound guidance to do so (confirm with trial site)
- You have any of the following additional conditions: end-stage kidney disease, a heart attack, a stroke, or active internal bleeding
- Your pain requires frequent or routine doses of strong IV pain medication
- You are unable to walk on your own to a bedside toilet
- The doctor believes you are likely to need a CT scan, MRI, an internal camera procedure (endoscopy), a blood transfusion, a heart stress test, or surgery
- You are considered at high risk of getting significantly worse quickly
- The home hospital programme is currently at its maximum capacity of 5 patients
