Who might be able to join this trial:

• People who have been diagnosed with COPD at a severe stage (classified as GOLD C or D) with lung function below a certain level (FEV1 less than 65%)

• People who are currently using a home breathing support device (NIV or CPAP) on an ongoing basis
• People whose main lung condition is a restrictive lung disease (such as obesity-related breathing problems or chest wall disease) — though there may be exceptions if the breathing problem is considered primarily obstructive by the treating doctor (confirm with trial site)
• People with a BMI (body mass index) over 35
• People taking sedative or pain medications at higher levels — specifically more than three sedating drugs or more than 20mg of morphine per day
• People who have been diagnosed with severe obstructive sleep apnoea (more than 30 breathing interruptions per hour during sleep, using French diagnostic criteria)
• People whose thinking or memory difficulties would prevent them from giving informed consent to participate
• People who are pregnant
• People with a smoking history of fewer than 10 pack-years (a measure of how much a person has smoked over their lifetime)
• People with a psychiatric illness requiring antipsychotic medication, those currently in treatment for drug or alcohol addiction, or those without a fixed home address after leaving hospital
• People with an unstable heart condition affecting the coronary arteries
• People under 18 years of age
• People who would be unable to follow the trial's requirements and schedule
• People whose life expectancy is less than 12 months due to a condition other than COPD
• People who spent more than 10 days in intensive care during their recent hospital stay
