Who might be able to join this trial:

• People who have had an acute brain injury — such as an ischemic or hemorrhagic stroke (including a burst aneurysm), traumatic brain injury, brain damage after cardiac arrest, or a brain tumour — that required admission to intensive care and breathing support through a tube in the throat
• People who have been on mechanical ventilation (a breathing machine) for more than 48 hours
• People aged between 18 and 75 years old
• People whose brain condition has become stable, with no dangerous pressure building inside the skull, and who need only minimal sedation
• People whose ability to respond to commands or move (as measured by a standard brain injury scale) remains significantly limited (confirm with trial site)
• People who have successfully passed a test showing they can breathe on their own for a period of time
• People for whom this would be the first attempt at removing the breathing tube

Who may not be able to join:

• People whose brain injury is located in the lower back part of the brain (posterior cranial fossa)
• People admitted because of prolonged seizures or an infection of the brain or surrounding tissues
• People with a spinal cord injury causing paralysis of the limbs
• People with ongoing, uncontrolled seizures or an uncontrolled brain/spinal infection
• People for whom a plan to limit medical treatment is already in place
• People with existing long-term breathing problems, such as those who use home oxygen, home breathing support machines, or have been diagnosed with a chronic lung condition or treated for sleep apnoea with a breathing device
• People who have already failed more than three breathing trials
• People with serious chest injuries (more than two broken ribs, a broken sternum, or injuries requiring open chest surgery)
• People who have surgery planned within the next 7 days
• People who have already had a tracheotomy (a surgical airway in the throat) or had their breathing tube removed outside of this study's process
• People with a known history of blockage or narrowing of the upper airway
• People who are pregnant or breastfeeding
• People who are legally protected under guardianship arrangements, or who are not covered by a social insurance system (confirm with trial site)
• People already participating in another clinical study involving mechanical ventilation or breathing machine weaning
