Who might be able to join this trial:
- Men aged 18 or older who previously had radiation treatment (either external beam radiotherapy or brachytherapy) for prostate cancer at certain stages, and whose cancer had not spread to distant parts of the body at the time of that treatment
- At least 2 years have passed since the end of the original radiation treatment
- Blood PSA levels have risen in a specific way (by 2 points above the lowest recorded level) indicating the cancer may have returned
- A specialised scan (called a PSMA PET/CT) shows no sign of cancer spread to lymph nodes or other parts of the body
- A biopsy guided by MRI has confirmed that the cancer has returned locally within the prostate gland or nearby seminal vesicles
- The area of cancer can be clearly identified on a specific type of MRI scan (confirm with trial site)
- Not currently receiving hormone therapy
- Testosterone levels in the blood are at or above a certain level (confirm with trial site)
- PSA levels are doubling at a rate of between 6 and 24 months
- Generally fit and active, with little or no limitation in daily activities (confirm with trial site)
- A bladder and bowel symptom score below a certain threshold (confirm with trial site)
- Urine flow rate of at least 10 ml per second
- Life expectancy of more than 5 years
- PSA level at the time of recurrence is 10 or below
Who may not be able to join:
- People where the shape of the pelvis or significant calcium deposits in the prostate may make treatment difficult (confirm with trial site)
- People who cannot safely receive spinal or general anaesthesia
- People with conditions that may prevent them from completing the treatment or follow-up visits, or where the treating cancer specialist believes participation is not appropriate — for example, due to significant language barriers
- People with inflammatory bowel disease
- People who cannot have an MRI scan at 3 Tesla strength (for example, due to certain metal implants or other contraindications)
- People whose kidneys are not functioning well enough, based on a blood test result below a certain level (confirm with trial site)
- People who already have significant urinary side effects rated at a serious level before the trial begins
