Who might be able to join this trial:

• People aged between 18 and 64 years old
• People who have been clinically diagnosed with a first or repeated stroke, either the type caused by a blocked blood vessel or the type caused by bleeding in the brain
• People who are living at home after being discharged from hospital following their stroke
• People who score above 20 on a standard memory and thinking test called the Montreal Cognitive Assessment
• People who score 3 or higher on a standard disability scale called the modified Rankin Scale, which measures how much help a person needs with daily activities (confirm with trial site)
• People who can speak and communicate in Cantonese
• People who are able to attend all of the sessions involved in the trial

Who may not be able to join:

• People whose stroke-like event was a transient ischaemic attack (sometimes called a "mini-stroke"), or who had bleeding between the brain and skull (subdural or epidural haemorrhage)
• People whose stroke was caused by a brain tumour or a head injury
• People who have a mental health condition such as depression, schizophrenia, bipolar disorder, or a personality disorder
• People who have difficulty speaking clearly or understanding conversations
• People who have previously taken part in a self-management programme
