Who might be able to join this trial:

• Men aged 18 years or older
• People who have been diagnosed with prostate cancer (a specific type called adenocarcinoma) that has been confirmed by a tissue sample, and who need radical radiotherapy (high-dose radiation treatment aimed at curing the cancer)
• People whose cancer has a Gleason score of 3+4 or 4+3 (also called Grade Group 2 or 3), which describes how the cancer cells look under a microscope
• People whose cancer is at stage T3a or lower based on MRI scan results, meaning the cancer has not spread extensively beyond the prostate
• People whose PSA level (a protein measured in the blood that can indicate prostate cancer activity) was below 25 ng/ml before starting hormone therapy
• People who will be receiving hormone therapy (called androgen deprivation therapy, or ADT) for at least 6 months alongside their radiotherapy, as part of standard treatment — with some flexibility considered on a case-by-case basis for up to 12 months
• People with a general health and activity level rated 0 to 2 on the WHO Performance Status scale, meaning they are able to carry out daily activities with at most some limitations
• People who are willing and able to understand and sign a consent form
• People who are willing and able to complete health and wellbeing questionnaires throughout the study

Who may not be able to join:

• People who cannot have an MRI scan — for example, due to a pacemaker, certain metal implants in the body, or severe claustrophobia (fear of enclosed spaces)
• People with a urinary symptom score (called IPSS) of 13 or higher, indicating more significant urinary problems
• People who have more than 100 ml of urine remaining in the bladder after urinating
• People whose prostate gland is larger than 80cc in volume
• People who have other health conditions that may increase the risk of serious side effects from treatment, or that may prevent long-term follow-up — for example, inflammatory bowel disease
• People who have had a full hip replacement on one or both sides, or other metal implants in the pelvis that interfere with certain MRI imaging techniques
• People who have previously had radiotherapy to the pelvic area
• People whose cancer requires 2 to 3 years of hormone therapy based on how advanced their disease is
• People who have had another type of cancer in the last 2 years — though some exceptions may apply, such as certain non-aggressive skin cancers, early-stage bladder cancer where follow-up checks have come back clear, or small kidney growths being monitored without active treatment (confirm with trial site)
