Who might be able to join this trial:

• People aged 18 to 55 years of either gender who have provided written agreement to take part
• People whose symptoms score above a certain threshold on a standard psychiatric scale (called the PANSS), with at least one significant positive symptom such as hallucinations or delusions
• People whose symptoms have not responded well enough after being treated with two different antipsychotic medications, each at a proper therapeutic dose for at least 6 weeks
• People who have been on a stable dose of an antipsychotic medication for more than one month and are able to continue that medication throughout the study
• People who are not currently taking part in another randomised controlled clinical trial

Who may not be able to join:

• People who have misused substances in the past month, or have had a substance dependence problem in the past 6 months (an exception may apply for people prescribed methadone for opiate withdrawal — confirm with trial site)
• People with a history of, or current, medical or neurological conditions that could cause an unstable health course, such as epilepsy
• People who are pregnant
• People who have a known allergy or sensitivity to the study drug acetazolamide (ACZ), a history of a kidney acid condition called renal hyperchloremic acidosis, a condition called Addison's disease or adrenal failure, or a type of eye condition called chronic closed angle-closure glaucoma
• People who have previously been treated with acetazolamide or have had a reaction to it in the past
• People who have an intellectual disability as defined by the DSM-5 diagnostic standard
