Who might be able to join this trial:
- You are 18 years old or older
- You can be male or female
- You have been diagnosed with psoriasis by a skin doctor (dermatologist) for at least 2 years (this applies to at least 30% of participants in the trial)
- You are willing and able to give your consent to take part
- You meet the "high-risk psoriasis" criteria, meaning you have had psoriasis for more than 2 years, it covers more than 3% of your body surface area, and a special ultrasound scan of your joints and tendons has shown certain findings above a specific threshold (confirm with trial site)
Who may not be able to join:
- You currently have joint pain due to inflammation, pain where tendons attach to bones (enthesitis), or swelling of a whole finger or toe (dactylitis), found during a physical examination
- You are currently taking certain medications that suppress the immune system (such as methotrexate or apremilast), or you have ever taken a biologic therapy (a type of medicine made from living cells)
- Your blood tests show certain markers associated with rheumatoid arthritis at medium or high levels (confirm with trial site)
- You currently have an active cancer
- You have a history of a painful condition affecting many joints, such as osteoarthritis, rheumatoid arthritis, gout, or similar joint conditions that could make it hard to tell if psoriatic arthritis is developing
- You have a condition that would make it unsafe to take the study drug (guselkumab), including a serious active infection or untreated latent (inactive) tuberculosis
- You have a known allergy or sensitivity to the study drug
