• You have been admitted to hospital due to a flare-up (worsening) of your COPD
• You have severe breathing problems, meaning your blood oxygen levels are very low — either already confirmed before this hospital stay, or confirmed during it — possibly alongside conditions such as too many red blood cells, high blood pressure in the lungs, or right-sided heart problems (confirm with trial site)
• You are covered by social security or an equivalent health insurance scheme

• If you are a woman who could become pregnant, you have been using effective contraception (such as the pill, a coil, or tubal ligation) for at least one month, and a blood pregnancy test has come back negative
• If you are a woman who has gone through menopause, this has been confirmed by at least 12 months without a period (not caused by medication)

Who may not be able to join:

• You are under 18 or over 85 years old
• You are already receiving long-term non-invasive ventilation (a breathing support machine used at home)
• You have been diagnosed with sleep apnea and are already being treated with a CPAP machine; or, if you have never been tested for sleep apnea, a screening process may be carried out, and you may not be eligible if the results show severe sleep apnea (confirm with trial site)
• Your body mass index (a measure of weight relative to height) is above 35
• You were admitted to hospital because of an active COVID-19 infection
• Your blood carbon dioxide levels are too high — either shown by a test in the past 6 months when stable, or remaining high after you leave hospital (confirm with trial site)
• You are currently pregnant (confirmed by a blood test) or breastfeeding
• You have a significant mental health condition or dementia that might make it difficult to follow the trial's requirements
• You have smoked fewer than 10 pack-years in your lifetime (a measure of how much and how long you have smoked — confirm with trial site)
• You are not expected to survive more than 12 months due to a condition other than COPD
• You are unwilling to receive high-flow oxygen therapy
• You are currently subject to a court or administrative order restricting your freedom, or you are under legal guardianship or supervision
