Who might be able to join this trial:

General criteria across both parts of the trial:

• Adults aged 18 or older who have been diagnosed with a specific type of glaucoma (called primary open-angle glaucoma or pseudoexfoliation glaucoma)
• People whose vision is at a certain minimum level (good enough to read a standard eye chart at a reasonable distance)
• People who have completed at least two reliable visual field tests (tests that map what a person can see across their full field of vision), with results meeting specific accuracy standards

Additional criteria for one part of the trial (SGNT):

• People who have been newly diagnosed and have not yet received any treatment for glaucoma in one or both eyes
• People whose glaucoma diagnosis is confirmed by specific changes to the optic nerve and/or vision

Additional criteria for the other part of the trial (VBIGS):

• People who already have a confirmed diagnosis of treated glaucoma (including normal tension glaucoma or pseudoexfoliation glaucoma) in both eyes
• People whose visual field loss falls within a moderate range (between -3 and -18 dB on testing)
• People who have had a certain type of laser eye treatment (selective laser trabeculoplasty) may still be eligible, provided it was performed 3 or more months before joining and liver function tests are normal
• People already taking a supplement called NAM may still be considered, but would need to stop taking it for one month before the study begins

Who may not be able to join:

For the SGNT part:

• People whose vision field damage is beyond a certain level of severity in either eye, or who have a specific type of deep blind spot in any eye
• People whose eye pressure is above a certain high level
• People who are pregnant or breastfeeding
• People who are not willing to avoid taking NAM (nicotinamide) supplements during the trial
• People who are allergic to NAM or niacin
• People who have been diagnosed with cancer in the past 5 years (except for certain treated skin cancers)
• People with a history of liver disease or stomach ulcers
• People with a condition that would prevent them from attending long-term follow-up appointments
• People with neurological or other non-glaucoma conditions (other than cataracts) that could affect visual field test results
• People who are unable to understand and speak Swedish or English
• People who have had eye surgery (other than uncomplicated cataract surgery)
• People with conditions known to affect the retina, such as more than mild age-related macular degeneration or more than early-stage diabetic eye disease

For the VBIGS part:

• People with a history of liver disease or gout
• People with cataracts that significantly affect vision
• People with other conditions that could affect visual field test results
• People who have had cataract surgery in the last 3 months
• People who have had glaucoma drainage surgery in the last 6 months
