Who might be able to join this trial:
- People aged 40 years or older
- People who have recently received treatment for a COPD flare-up (worsening of symptoms), either in a hospital or through outpatient care
- People who have been referred to a pulmonary rehabilitation (lung exercise and education) program
- People who are able to understand and communicate in English
- People who are willing to take part in phone calls with a peer coach and watch storytelling videos as part of the study
- People who have access to a working phone
Who may not be able to join:
- People who are unwilling to attend pulmonary rehabilitation
- People who are not considered medically suitable for pulmonary rehabilitation based on breathing tests or other health reasons, as determined by pulmonary rehabilitation staff
- People who are currently enrolled in pulmonary rehabilitation, or who have already completed 12 or more sessions of pulmonary rehabilitation in the past
- People who are receiving comfort-focused or hospice (end-of-life) care
- People who live in a long-term care or nursing facility
- People who are unable or unwilling to provide informed consent to participate
