Who might be able to join this trial:

For stroke patients:

• People who were sent home from NINSH hospital after a stroke, or who visited NINSH shortly after having a stroke
• People who are having their first-ever stroke (not a repeat stroke)
• People who have had any type of stroke, except a specific type called subarachnoid haemorrhage
• People whose stroke-related disability falls within a certain range on a standard medical scale (modified Rankin Scale score of 0–4, meaning some ability to function independently — confirm with trial site)
• People aged 18 years or older
• People of any gender
• People who are willing to take part and have signed a written consent form

For family caregivers:

• The main caregiver of an eligible stroke patient, of any gender, aged 18 or older, who lives with the patient and provides their care
• People who are willing to take part and have signed a written consent form

Who may not be able to join:

For stroke patients:

• People who have had a stroke before (this would not be their first stroke)
• People whose stroke was caused by a genetic condition or by a physical injury or accident
• People with failure of multiple major organs, or who are in the final stage of a terminal illness
• People who are already taking part in another clinical trial
• People who do not wish to participate
• People whose household does not have access to a mobile phone
• People with a very high level of stroke-related disability on the standard medical scale (modified Rankin Scale score of 5 — confirm with trial site)

For family caregivers:

• Caregivers who do not live in the same home as the patient
• People under 18 years of age
• People who have a diagnosed mental health condition or a diagnosed condition affecting memory and thinking
