Who might be able to join this trial:
For stroke patients:
- People who were sent home from NINSH hospital after a stroke, or who visited NINSH shortly after having a stroke
- People who are having their first-ever stroke (not a repeat stroke)
- People who have had any type of stroke, except a specific type called subarachnoid haemorrhage
- People whose stroke-related disability falls within a certain range on a standard medical scale (modified Rankin Scale score of 0–4, meaning some ability to function independently — confirm with trial site)
- People aged 18 years or older
- People of any gender
- People who are willing to take part and have signed a written consent form
For family caregivers:
- The main caregiver of an eligible stroke patient, of any gender, aged 18 or older, who lives with the patient and provides their care
- People who are willing to take part and have signed a written consent form
Who may not be able to join:
For stroke patients:
- People who have had a stroke before (this would not be their first stroke)
- People whose stroke was caused by a genetic condition or by a physical injury or accident
- People with failure of multiple major organs, or who are in the final stage of a terminal illness
- People who are already taking part in another clinical trial
- People who do not wish to participate
- People whose household does not have access to a mobile phone
- People with a very high level of stroke-related disability on the standard medical scale (modified Rankin Scale score of 5 — confirm with trial site)
For family caregivers:
- Caregivers who do not live in the same home as the patient
- People under 18 years of age
- People who have a diagnosed mental health condition or a diagnosed condition affecting memory and thinking
