Who might be able to join this trial:
- People who are willing to sign and date a consent form agreeing to take part in the study
- People who are willing to take part in the COPD Wellness program
- People between 40 and 90 years old
- People who speak English or Spanish
- People who have been diagnosed with COPD by a doctor

- People who are currently receiving care within the San Francisco Health Network (SFHN)
Who may not be able to join:
- People who are pregnant
- People living with dementia, significant memory or thinking difficulties, or active psychiatric symptoms that would make participation difficult
- People with unstable heart or blood vessel conditions, including a heart attack or blood clot in the lungs within the past 6 months, an uncontrolled irregular heartbeat, or poorly managed heart failure
- People with another serious health condition where exercise would not be considered safe (this would be assessed by a registered nurse working with a lung specialist)
- People with an active contagious lung infection such as tuberculosis or COVID-19
- People who have taken part in a pulmonary rehabilitation program within the past 12 months
- People who have had a COPD flare-up within the past 6 weeks
- People with physical limitations that prevent them from doing moderate physical activity
- People with a diagnosis or condition where life expectancy is less than approximately one year
