Who might be able to join this trial:
For the first part of the study (Diagnostic Arm):
- People who are Veterans and currently receive their healthcare through the VA system
- People who have had ongoing pain in their shoulder, hip, or knee for at least one year
- People whose painful joint shows mild to moderate wear-and-tear changes on a previous X-ray, CT scan, or MRI
- People who have already tried at least one treatment for their joint pain (such as physical therapy, anti-inflammatory medications, or steroid injections) without enough relief
- People who have had an MRI of their painful joint ordered by a healthcare provider
For the second part of the study (Therapeutic Arm):
- People who completed the first part of the study and had a special type of MRI that measures blood flow around the joint
- People whose blood flow MRI shows increased blood vessel activity around the painful joint, as determined by the study's lead researchers
Who may not be able to join:
For the first part of the study (Diagnostic Arm):
- People who have had an injury to the affected joint within the last 12 weeks
- People under 25 years of age
- People whose joint shows severe wear-and-tear changes on imaging
- People who have had a joint replacement or a metal implant placed inside the painful joint
- People with a severe allergy to gadolinium, the contrast dye used in certain MRI scans
- People with serious kidney disease or who are currently on dialysis
- People with a history of cancer with a life expectancy of less than one year, or cancer that has spread to the bones
- People who are currently pregnant
- People who have a formal diagnosis of a cognitive (thinking or memory) impairment
For the second part of the study (Therapeutic Arm):
- People with a severe allergy to iodine-based contrast dye used in certain scans
- People with serious heart artery or limb artery disease
- People who have a blood clotting disorder
- People with poorly controlled type 2 diabetes or diabetes-related nerve damage
- People with a history of seizures or serious reactions to light sedation (medication used to help patients relax during procedures)
- People who have previously had bypass surgery on an artery in their arm or leg
- People currently participating in another treatment-based clinical trial for chronic joint pain
- People whose blood flow MRI did not show increased blood vessel activity around the painful joint
- People who meet any of the exclusion criteria listed for the first part of the study
