Who might be able to join this trial:

• You are willing to sign a consent form agreeing to take part in the study
• You are 18 years of age or older
• You have been diagnosed with advanced or spread-to-other-areas esophageal, stomach, liver, pancreatic, bile duct, colorectal, or lung cancer, confirmed by a tissue sample, within the last 12 weeks
• You have lost a significant amount of weight — either 5% or more of your body weight over the past 6 months, or 2% or more if you also have a low body weight or low muscle mass
• You are about to start or have recently started your first round of cancer treatment (such as chemotherapy, targeted therapy, or immunotherapy, with or without radiation), and have not yet begun a second cycle of that treatment
• You are able to walk on your own, with or without a walking aid such as a cane or walker
• If you have cancer that has spread to the brain, it must either cause no symptoms or have already been treated, with any side effects resolved, and you must have been off steroids and neurologically stable for at least 4 weeks before joining
• Your general health and ability to carry out daily activities falls within an acceptable range as measured by a standard scale used in cancer care (confirm with trial site)
• You are willing and able to stop taking any medications or over-the-counter products that could interact with the study drug
• You are willing to avoid breastfeeding during the study
• If you could become pregnant or father a child, you are willing to use contraception during the study and for one month after, and agree not to donate eggs or sperm during this time
• If you could become pregnant, you must have a negative pregnancy test at the start of the study

Who may not be able to join:

• You are taking or planning to take certain antipsychotic medications (such as risperidone, quetiapine, clozapine, or similar drugs) within 30 days of joining — note that some anti-nausea medications in the same drug family may still be allowed (confirm with trial site)
• You have been taking appetite-stimulating medications or supplements in the past 14 days or more, such as megestrol acetate, cannabis-based products (including medical cannabis or over-the-counter cannabinoid products), or higher-dose steroids (unless they are being used for standard chemotherapy-related nausea prevention)
• You have poorly controlled diabetes, meaning your blood sugar levels or a specific blood sugar control test (HbA1c) are above certain levels (confirm with trial site)
• Your liver or kidneys are not working well enough, based on recent blood test results — this includes being on dialysis or having very low kidney function
• You are currently being fed through a feeding tube or directly into a vein (IV nutrition)
• You have a condition that makes it hard to swallow or absorb the study drug through your digestive system, such as difficulty swallowing, mouth sores, removal of part of the stomach, bowel blockage, or a similar condition
• You have recurring fluid build-up in your abdomen that has not responded to treatment and requires regular draining
• You have symptoms that are not well controlled and make you unsuitable for the study, in the opinion of the study doctor — though if symptoms are managed well for at least one week, you may still be considered
• You have an active, uncontrolled infection, including COVID-19, at the time you would start the study
• You have another significant medical or mental health condition, including thoughts of suicide within the past year or currently, that the study doctor believes would make participation unsafe or unsuitable for you
