Who might be able to join this trial:

• Children and teenagers aged 1 to under 18 years old.
• People who have been diagnosed with a first relapse (the cancer has come back for the first time) of a specific type of leukaemia called B-cell precursor ALL, classified as either high-risk or very high-risk based on when it came back and certain genetic features of the cancer.
• The leukaemia cells must test positive for a protein called CD22, as determined by the treating hospital.
• At least 5% of the bone marrow must contain leukaemia cells at the time of joining.
• Kidney function must be at or above a certain level, measured by a blood test (confirm with trial site for the exact number required for the child's age group).
• Liver enzyme levels (AST and ALT) must be no more than 5 times the upper limit of normal at the hospital where treatment takes place.
• Bilirubin levels (a measure of liver function) must be within an acceptable range, unless the child has a condition called Gilbert's syndrome.
• Children who have previously had blood clots while on steroids or a medicine called asparaginase may still be eligible, provided they receive blood-thinning medication as a preventive measure during the trial.
• The heart must be functioning well enough, as shown by a specific heart scan result (confirm with trial site for exact measurements).
• Children whose leukaemia has returned in both the bone marrow and the testicles may be eligible, provided surgical removal of the affected testicle has been done or is planned.

• People who have previously received a stem cell transplant from a donor, or a treatment called CAR T-cell therapy.
• People whose leukaemia has returned only outside the bone marrow (for example, only in the brain or other organs), with no bone marrow involvement.
• People whose leukaemia has a specific genetic abnormality called Philadelphia chromosome positive (BCR-ABL).
• People who have previously been treated with a type of antibody-based medicine containing a substance called calicheamicin (such as inotuzumab ozogamicin or gemtuzumab ozogamicin).
• People who currently have an active infection (bacterial, fungal, or viral) that is not under control.
• People who are allergic to both forms of a medicine called asparaginase (PEG-ASP and Erwinia-ASP).
