Who might be able to join this trial:

For people with a schizophrenia spectrum disorder (experimental group):

• People aged 17 to 45 years
• People of any sex
• People who have been diagnosed for the first time with one of the following conditions: schizophrenia, schizoaffective disorder, schizophreniform disorder, delusional disorder, brief psychotic disorder, a psychotic disorder not otherwise specified, major depressive disorder with psychotic symptoms, bipolar I or II disorder, or substance-induced psychosis — and who are receiving antipsychotic medication for one of these conditions (diagnosis will be confirmed through medical records)
• People with a Body Mass Index (BMI) of 27 or below (BMI is a measure based on height and weight)

For healthy volunteers (control group):

• People aged 17 to 45 years
• People whose sex, dominant hand, and BMI are matched to a participant in the experimental group
• People who do not have any condition listed in the DSM-5 diagnostic manual, other than a specific phobia (a fear of a particular thing or situation)

Who may not be able to join:

• People with moderate or severe substance use disorder (the trial team will use their judgment in cases of moderate alcohol or cannabis use)
• People who have prediabetes, diabetes, or signs of difficulty processing blood sugar, identified through a glucose tolerance test at screening
• People who are currently at acute risk of suicide
• People who are currently taking medications for weight management, cholesterol, diabetes, or blood pressure
• People with liver or kidney disease
• People who are pregnant
• People who are currently breastfeeding
• People who have a pacemaker
• People who have a metallic heart valve
• People who have magnetic materials in their body, such as surgical clips or implanted electronic pumps, or any other condition that would make an MRI scan unsafe
• People with significant claustrophobia (fear of enclosed spaces), as determined by the MRI screening form and the study team's assessment
• People with a history of a head injury that caused loss of consciousness for more than 30 minutes and required medical treatment
• People whose head or neck size would prevent them from fitting in the MRI or PET scanning equipment
• People who weigh more than 350 pounds (the weight limit for the MRI scanner bed)
• People with an unstable physical health condition
• People with a significant neurological condition, including epilepsy or a seizure disorder
• People who have already received the maximum allowed radiation dose of 20 mSv in the past 12 months
• People who have had more than 6 PET scans in their lifetime, if participating in this study would bring their total above 8 PET scans lifetime (the limit set by the Centre for Addiction and Mental Health Brain Health Imaging Centre)
• People with a clinically significant abnormality noted in their medical history (confirm with trial site)
• People currently taking any medication that could interfere with the study or affect the results of PET imaging (for example, anti-diabetic medications)
• People with blood clotting disorders or who are currently taking blood-thinning medications

Additional exclusion for healthy volunteers only:

• People who have a parent, sibling, or child with a primary psychotic disorder
