Who might be able to join this trial:

• People aged 18 or older
• People whose general health and ability to carry out daily activities meets a certain minimum level, as assessed by a standard medical scale called the ECOG Performance Status (score must be below 2) (confirm with trial site)
• People who have already been diagnosed with prostate cancer with a Gleason score (a measure of cancer aggressiveness) above 6

For Cohort A (intermediate-risk prostate cancer, no prior treatment):

• People with intermediate-risk prostate cancer as classified by current national cancer guidelines
• People with greater than a 10% calculated chance of the cancer having spread to nearby lymph nodes, based on a recognised medical calculator
• People who have had a recent prostate biopsy and a specialised prostate MRI scan, both within the past 12 weeks
• People who have not previously received any treatment for prostate cancer
• People in the unfavorable intermediate-risk group who have also had a CT scan of the chest, abdomen and pelvis, and a bone scan, within the past 12 weeks

For Cohort B (high or very high-risk prostate cancer, no prior treatment):

• People with high or very high-risk prostate cancer as classified by current national cancer guidelines
• People who have had a recent prostate biopsy and a specialised prostate MRI scan, both within the past 12 weeks
• People who have not previously received any treatment for prostate cancer
• People who have also had a CT scan of the chest, abdomen and pelvis, and a bone scan, within the past 12 weeks

For Cohort C (prostate cancer that has returned or spread after earlier treatment):

• People whose PSA level did not drop to undetectable levels after surgical removal of the prostate, or whose PSA rose again on two or more tests after initially becoming undetectable
• People whose PSA level has risen by 2 ng/mL or more above its lowest recorded point after radiation therapy
• People who have imaging evidence of cancer that has spread, even without a rising PSA
• People with physical symptoms suggesting the cancer has spread to distant parts of the body

Who may not be able to join:

• People who do not have a confirmed tissue-based diagnosis of prostate cancer
• People with a pacemaker or metal implants that would prevent them from safely having a pelvic MRI scan
• People with significantly abnormal liver function, including highly elevated liver enzymes or bilirubin levels above certain thresholds
• People with significantly reduced kidney function (eGFR below 30 ml/min)
• People who have had a heart attack, a stroke, a mini-stroke (TIA), an unstable heart condition requiring hospitalisation, a blood clot in the lungs, or a deep vein blood clot within the past 6 months
• People with acute heart failure or a serious heart rhythm problem, including certain types of electrical conduction problems in the heart
• People with an active infection that the treating doctor considers serious enough to affect participation, such as a urinary tract infection, respiratory infection, or a severe bone infection
