Who might be able to join this trial:

• People aged between 18 and 80 years old
• People who have been diagnosed with a brain bleed (haemorrhage) in a specific area of the brain called the basal ganglia, confirmed by a CT scan
• People whose brain bleed is between 20 and 40 millilitres in size, with less than 3mm of shift in the central structures of the brain
• People whose brain imaging (a special MRI scan called diffusion tensor imaging) shows a specific level of damage to the nerve pathway that controls movement, rated as grade 2 to 4 (confirm with trial site)
• People who are enrolled in the trial within 24 hours of their symptoms starting
• People who score 4 or above on a standard level-of-consciousness assessment called the GCS (Glasgow Coma Scale) at the time of enrolment
• People who have some movement remaining in the affected arm or leg, measured at a specific level called muscle strength grade 3
• People who, or whose legal representative, are able to provide written consent to participate

Who may not be able to join:

• People whose brain bleed is located in a different area, such as the thalamus, brainstem, or cerebellum
• People whose brain bleed was caused by another underlying condition, such as a burst aneurysm, abnormal blood vessels, head injury, brain tumour, stroke, a protein build-up disorder, or a blood clotting problem
• People who have more than one area of bleeding in the brain
• People whose bleed has broken through into the fluid-filled spaces of the brain (ventricles) and who may need a drain inserted
• People who have had any brain bleed or brain surgery in the 30 days before joining the trial
• People who have had a heart attack in the past 30 days
• People with a history of bleeding elsewhere in the body, such as the digestive system, urinary system, or airways, that has not fully stopped
• People with low blood counts, including low haemoglobin, low red blood cell volume, or low platelet levels (confirm with trial site for specific thresholds)
• People who have been taking blood-thinning medications such as warfarin, dabigatran, or rivaroxaban in the week before joining, and whose blood-clotting level is above a certain threshold
• People who are expected to need long-term blood-thinning or anti-clotting medication
• People with a known allergy to certain clot-dissolving medications (alteplase or urokinase) or to any equipment or medications used in surgery
• Women who are pregnant or breastfeeding
• People with certain heart conditions that carry a high risk of blood clots, such as a mechanical heart valve, blood clots in the heart, a specific type of heart valve narrowing with an irregular heartbeat, acute inflammation around the heart, or a serious heart infection
• People whose blood pressure remains very high (above 180mmHg) even after receiving strong blood pressure-lowering treatment before enrolment
• People with an advanced illness where life expectancy is less than 12 months
• People who are already taking part in another clinical trial involving a treatment or intervention
