Who might be able to join this trial:
- People who have had an acute stroke caused by a blocked blood vessel in the back part of the brain (posterior circulation), confirmed by symptoms and brain scans.
- People whose stroke is caused by a blockage in a specific artery at the base of the brain (called the basilar artery), confirmed by a specialised blood vessel imaging scan.
- People aged 18 years or older.
- People whose stroke symptoms started no more than 24 hours before being enrolled in the trial.
- People with a stroke severity score of 10 or higher on a standard medical scale (NIHSS) before enrolment, indicating a moderately severe to severe stroke.
- People whose hand has normal blood supply through two separate pathways, confirmed by a simple physical test (called the modified Allen's test).
- People (or their legally authorised representative) who are able to provide written agreement to participate.
Who may not be able to join:
- People who already had significant disability before the stroke, based on a standard disability scale (mRS score of 3 or higher).
- People who are pregnant or breastfeeding.
- People who have a known allergy to contrast dye used in scans, or to a metal called nitinol used in medical devices.
- People who are already taking part in another clinical trial.
- People with very high blood pressure (systolic above 185 mmHg or diastolic above 110 mmHg) that cannot be brought under control with blood pressure medication.
- People with a known condition that affects blood clotting, or who are taking blood-thinning medication with a clotting test result (INR) above 1.7.
- People with blood test results outside safe limits, including very low or very high blood sugar, a low platelet count, or low red blood cell levels (confirm specific values with trial site).
- People who are not expected to live longer than one year.
- People who are unlikely to be available for follow-up appointments within 90 days, for example due to having no fixed address or living overseas.
- People who had their stroke within 48 hours of a heart procedure, brain blood vessel procedure, or major surgery (those more than 48 hours post-procedure may still be considered).
- People showing signs of inflammation of the blood vessels in the brain (central nervous system vasculitis).
- People with pre-existing neurological conditions or mental health disorders that would make it difficult to assess the stroke's effects.
- People with physical conditions or abnormalities that would make it difficult to access the arteries in the wrist or groin needed for the procedure, such as local infection, structural abnormalities, or previous surgery in those areas.
