Who might be able to join this trial:
- People aged between 20 and 40 years
- People with a body mass index (a measure of body weight relative to height) between 18 and 35
- People diagnosed with polycystic ovary syndrome (PCOS) based on specific recognised criteria, which includes irregular or absent periods, signs of higher-than-normal male hormones, and polycystic ovaries seen on an ultrasound
- People with a mildly underactive thyroid, meaning a specific thyroid hormone level (TSH) is slightly elevated (between 5–10 mIU/L) while another thyroid hormone (free T4) remains in the normal range
- People who have been trying to conceive without success for at least one year

- People using effective contraception if they are not currently trying to conceive

Who may not be able to join:
- People who have a previously known thyroid condition or are currently taking thyroid medication
- People with thyroid test results that are abnormal in ways other than the mild elevation described above
- People with elevated levels of the hormone prolactin (hyperprolactinemia)
- People whose infertility is related to other specific causes, such as moderate to severe male fertility problems, both fallopian tubes being blocked, stage III or IV endometriosis, or ovarian failure or low ovarian reserve
- People who have previously been diagnosed with congenital adrenal hyperplasia (a hormonal condition present from birth)
- People with poorly controlled diabetes, indicated by a specific blood sugar marker (HbA1c) above 8%
- People with a history of blood clots in the veins (such as deep vein thrombosis) or related conditions
- People for whom either of the two study medications — letrozole or levothyroxine — would not be medically appropriate (confirm with trial site)
- People who have used letrozole or levothyroxine within the past 6 months
- People who are currently pregnant or where pregnancy is suspected
- People who are currently breastfeeding
- People who would be unable to follow the treatment plan and attend required follow-up appointments
