Who might be able to join this trial:

• People who are 60 years of age or older
• People who are able to carry out everyday tasks on their own (such as dressing, eating, and moving around)
• People with COPD who have noticeable symptoms (measured by specific breathing questionnaires) or who have not been regularly active at a moderate or high level over the past year, meaning physical activity levels fall below World Health Organization recommendations
• People with COPD whose diagnosis has been confirmed by a breathing test (called spirometry) taken after using a bronchodilator inhaler, showing a specific result that indicates airflow limitation
• People without any health conditions who have normal lung function (eligible to join as healthy comparison participants)

Who may not be able to join:

• People who have unstable or serious movement, heart, nerve, muscle, bone, or joint conditions that would prevent them from safely doing exercise or tests, as determined by a doctor — this includes things like severe musculoskeletal pain, a recent heart attack or heart procedure, uncontrolled high blood pressure, or abnormal heart readings during an exercise test
• People who have other lung conditions, such as asthma, interstitial lung disease, lung cancer, pulmonary hypertension, or pulmonary fibrosis
• People who have medical conditions or are receiving treatments known to affect brain function or thinking, including past head injuries with lasting effects, neurological conditions (such as dementia, multiple sclerosis, or stroke), serious psychiatric illness (not including depression or general anxiety disorder), severe thinking or memory difficulties, or a recent or current cancer diagnosis and treatment
• People with physical or mental disabilities that would affect participation (confirm with trial site)
• People who have metal implants, pacemakers, claustrophobia, or other factors that make MRI or PET/CT scanning unsafe — this applies to those agreeing to those specific scans
• People who are unable to read or speak Swedish (for participants at the Umeå site), Dutch or French (for participants at the Hasselt site), or English (for participants at either site)
• People with COPD whose ability to exercise is limited more by another health condition than by their COPD itself
• People with COPD who are currently taking part in, or have taken part in, a pulmonary rehabilitation program within the last 12 months
• People with COPD who have had a flare-up (exacerbation) that led to a change in their medication within the past six weeks
