Who might be able to join this trial:
- Adults of any sex, aged over 35, who are not suitable candidates for total knee replacement surgery due to age (too young or too old), other health conditions, or technical reasons related to their anatomy or situation
- People who score below 30 on the Oxford Knee Score (OKS), a standard questionnaire measuring how well the knee is functioning
- People experiencing moderate to severe ongoing knee pain — scoring 6 or higher on an 11-point pain scale — either constantly or during movement, despite having tried at least 3 months of conservative treatments such as physiotherapy, pain medications (paracetamol or anti-inflammatories), or steroid injections into the knee joint
- People with imaging (X-ray or MRI) confirming knee arthritis, rated at a Kellgren-Lawrence score of 2 or higher
Who may not be able to join:
- People who have previously had nerve ablation (a procedure to deaden nerves) around the knee, or who have had any prior partial, resurfacing, or total knee replacement surgery on the affected knee
- People with a history of nerve or blood vessel injury near the knee, or a recent injury to the lower limbs
- People who experience widespread chronic pain throughout their body
- People with nerve damage affecting multiple nerves (polyneuropathy) or nerve pain radiating down the leg from the spine
- People who currently have a nerve stimulator device implanted in their body
- People with an active local or widespread infection, including infection in the bloodstream
- People with poorly controlled immune system suppression
- People who have received any injection into the affected knee joint (such as steroids, hyaluronic acid, platelet-rich plasma, or stem cells) within the 90 days before joining the trial
- People who have had arthroscopic knee surgery (a camera-guided procedure to clean or wash out the joint) within the 180 days before joining the trial
- People with a very low BMI (under 18.5 kg/m²) or with very little tissue under the skin around the knee, which could make certain treatment procedures unsafe due to risk of skin burns
- People with known allergies to any substances used during the procedure, including local anaesthetic (lidocaine), phenol, or contrast dye
- People who have already agreed on and scheduled a date for total knee replacement surgery
- People identified by the trial investigators as having significant psychosocial difficulties that may affect participation
