Who might be able to join this trial:
- Men who have been diagnosed with metastatic prostate cancer (a type called adenocarcinoma, where the cancer has spread beyond the prostate)
- People currently receiving hormone-blocking treatment (androgen deprivation therapy), either alone or combined with one of these medications: abiraterone, enzalutamide, darolutamide, or apalutamide — and who have been on that treatment for at least 3 months
- People whose PSA level is above 1 ng/mL, or whose PSA has increased across at least two blood tests taken at least 2 weeks apart
- People who have had any number of previous cancer treatments, including chemotherapy or radium-223
- People whose blood test for a specific cancer marker (called AR-V7, tested at Hopkins laboratory) shows a positive result
- People who, if starting a bone-strengthening medication, have begun it at least 2 weeks before the exercise program starts
- People with a general physical fitness level rated as fully active or able to carry out light work (ECOG score of 0 or 1)
- People aged 80 years or under
- People currently under the care of a heart specialist (seen in the past year) who have received clearance from that specialist to participate
Who may not be able to join:
- People whose prostate cancer is a specific rare type called small cell carcinoma
- People who are unable to take part in or keep track of an exercise program due to conditions such as memory or thinking difficulties
- People experiencing moderate to severe bone pain that affects everyday activities, including those using strong pain medications (narcotics) for prostate cancer-related pain
- People whose cancer is progressing in a way that requires a change in their current drug treatment, or who need radiation therapy to manage symptoms
- People who have bone cancer spread to the spine or long bones where the treating doctor considers referral to a radiation or surgical specialist necessary due to fracture risk
- People with serious heart or cardiovascular conditions, or other significant health problems, that would make 12 weeks of exercise unsafe (confirm with trial site)
- People who have had major surgery within the past 4 weeks
