- You were recently discharged from UHN hospital after a flare-up of COPD (a sudden worsening of symptoms), and have been referred to either a Virtual Pulmonary Rehabilitation program or Remote Clinical Monitoring at Toronto Grace Health Centre

- Your main breathing or lung condition is something other than COPD, such as bronchiectasis or asthma
- Your medical team has estimated that your life expectancy is 2 months or less
- You have significant memory or thinking difficulties, and you do not have a family caregiver who is able to give consent and help follow the study steps on your behalf
