Who might be able to join this trial:
- People aged 30 to 85 years at the time of signing consent
- People who have weakness on one side of the body caused by one or more strokes (either a blocked blood vessel or a bleed in the brain)
- People whose most recent stroke (for which they received treatment) happened at least 6 months before joining the study
- People whose walking speed is slower than 1.0 metre per second on a standard 10-metre walking test
- People who can walk 10 metres on their own — with a walking aid if needed — without needing another person to physically support them the whole time (a helper standing nearby or giving occasional balance support is allowed)
- People who can walk on a treadmill for at least 3 minutes at a very slow speed (0.13 metres per second or 0.3 miles per hour)
- People who have a stable heart condition that falls within a specific medical category (known as AHA Class B — confirm with trial site for details)
- People who are able to have a conversation with the research team, follow a two-step instruction, and correctly answer questions to show they understand what they are consenting to
Who may not be able to join:
- People whose heart activity at rest makes it impossible to check for heart problems during exercise testing, unless recent heart test results from the past year are available to rule those problems out
- People who show signs of a significant irregular heartbeat or reduced blood flow to the heart during a treadmill exercise test, unless more detailed heart testing from the past year showed a clear result
- People who were admitted to hospital for a heart or lung condition within the past 3 months
- People with an implanted pacemaker or defibrillator that has an upper heart rate limit that would interfere with exercise testing or exercise prescription, or where the limit is unknown
- People with significant problems with balance/coordination or with ignoring one side of their body, based on a standard stroke assessment score (confirm with trial site)
- People with severe muscle stiffness in the lower limbs, based on a standard stiffness scale (confirm with trial site)
- People with a known recent history (within the past 3 months) of substance misuse or mental illness that is not stable
- People with significant depression after their stroke (based on a standard questionnaire score) who are not currently receiving depression treatment from a healthcare provider
- People who are currently taking part in physiotherapy or another study aimed at improving walking
- People who have recently changed (within 2 weeks) or are planning to change their leg brace or muscle stiffness treatments
- People who have foot drop or unstable leg joints without an appropriate supportive device, as assessed by a physiotherapist
- People with a significant neurological condition other than stroke
- People who were unable to walk outside the home before their stroke
- People with another significant medical condition that a physiotherapist determines is likely to limit improvement or affect safety (for example, joint stiffness or pain that limits walking)
- People who are pregnant
- People who have had more than 3 hours total of fast treadmill walking in the past year
