Who might be able to join this trial:

• People aged 19 years or older
• People who have a significant narrowing of the carotid artery (a major blood vessel in the neck) — either more than 50% narrowing with symptoms, or more than 70% narrowing without symptoms — and who are planned to have or have already had a stent placed in that artery
• People who are considered at high risk of serious bleeding, based on an overall risk score, AND who meet at least one of the following specific conditions:
  • A serious bleeding episode (not from a procedure site, such as from the gut or urine) in the 12 months before the stent procedure
  • A history of serious bleeding that has not been fully treated or resolved
  • Age 75 years or older
  • A low platelet count (below 100,000/mm³), based on screening blood tests — platelets are cells that help blood clot
  • A blood clotting disorder that increases bleeding risk (such as Von Willebrand disease, or certain clotting factor deficiencies)
  • Low red blood cell levels (anaemia), defined as haemoglobin below 12g/dL in men or below 11g/dL in women, or having donated blood within the past 4 weeks
  • Having taken steroids or anti-inflammatory medicines (such as ibuprofen-type drugs) for 4 weeks or longer
  • An active cancer diagnosis, other than a common type of non-serious skin cancer
  • Kidney disease, including being on dialysis, having had a kidney transplant, or having reduced kidney function based on a specific blood measurement
  • Liver disease involving scarring of the liver (cirrhosis) with associated high blood pressure in the liver's blood vessels
  • Five or more tiny bleeds in the brain detected on a brain scan
  • A stroke or mini-stroke (TIA) within the past 6 months, or a condition involving brief episodes of vision loss in one eye
  • Any past bleeding inside the brain that was not caused by injury, or a brain bleed caused by injury within the past 12 months

Who may not be able to join:

• People who have had a serious cardiovascular, stroke-related, or major bleeding event within 30 days after their carotid artery stent procedure
• People who stopped taking their two-drug blood-thinning tablet combination (dual antiplatelet therapy) within 30 days of the stent being placed — a brief switch to one medication for up to 7 days due to infection or injury may be allowed, but two medications must be in use again by 28–30 days after the stent (confirm with trial site)
• People who have had a stent placed in a heart artery or another blood vessel, or have had a procedure to re-open a blocked blood vessel, within the past year — or who need a type of heart bypass surgery or another treatment requiring two blood-thinning tablets
• People with a known allergy or sensitivity to aspirin or clopidogrel (common blood-thinning medications)
• Women who are pregnant or breastfeeding — women who could become pregnant will need a pregnancy test before joining, and will need to use reliable contraception throughout the trial
• People who need to take blood-thinning medication (anticoagulation) for 12 months or more
• People who need to take other types of antiplatelet (blood-thinning) medications beyond those used in the trial
• People who are currently taking part in another clinical trial involving a treatment or intervention
