Who might be able to join this trial:

• You have been diagnosed with prostate cancer confirmed by a tissue biopsy (if the biopsy was done more than 12 months ago, a new one may be needed)
• Your prostate cancer is localised (has not spread to lymph nodes or other parts of the body), and is classed as high-risk, very high-risk, or unfavourable intermediate-risk (confirm with trial site)
• You have given written agreement to take part in this specific trial
• You are over 18 years old
• You have already had both a PSMA-PET scan and a special prostate MRI scan (mpMRI), or a combined PSMA-PET/MRI scan, meeting required standards
• You are generally well and able to carry out normal daily activities with little or no limitation (confirm with trial site)
• Your urinary symptom score is 15 or below on a standard questionnaire (confirm with trial site)
• Your prostate is no larger than 75 ml in size at the time of treatment planning

Who may not be able to join:

• Your biopsy shows a rare type of prostate cancer cells called neuroendocrine tumour cells
• You have previously had radiotherapy to your prostate or the pelvic area
• You have previously had your prostate surgically removed
• You have previously had any other local treatment directed at part of your prostate
• If you have started hormone therapy (ADT), more than one month passed between starting it and having your MRI and PSMA-PET scans
• Your scans show cancer has spread to nearby lymph nodes
• Your scans show cancer has spread to other parts of the body
• Your scans show the cancer has grown into nearby organs beyond the prostate (cT4 disease) (confirm with trial site)
• Your PSA level was higher than 30 ng/ml before starting hormone therapy
• Your life expectancy is estimated to be less than 5 years
• You have hip replacements on both sides, or other implants that would significantly affect CT scan image quality
• You have a medical reason why you cannot have an MRI scan (for example, certain metal implants or a pacemaker)
• You have had certain prostate surgery (TURP or HoLEP) that left a significant tissue cavity, or you have had this type of surgery within the last 6 months
• You have a medical condition that could make radiotherapy unsafe, such as active inflammatory bowel disease or paralysis of the limbs
• You have had another cancer in the last 2 years (except certain non-melanoma skin cancers), or a previous cancer that could significantly affect your 5-year survival
• You have any other medical reason that would make external beam radiotherapy to the pelvis unsuitable
• No visible tumour can be identified on your MRI or PSMA-PET scans
• You have taken part in another interventional clinical trial in the last 30 days
• You are currently taking part in another interventional clinical trial that could interfere with this one
• You are not able to understand or make decisions about your participation in the trial
• You have a known ongoing problem with misuse of medication, drugs, or alcohol
• There are concerns that you would have significant difficulties with the positioning required for treatment (confirm with trial site)
• The required radiation dose limits to protect surrounding healthy organs cannot be safely met
