Who might be able to join this trial:
- People who are 40 years of age or older
- People who have been admitted to hospital for a flare-up of their COPD (chronic obstructive pulmonary disease) and have recently been discharged from hospital care, including from virtual ward support (the hospital stay must have lasted at least 4 hours, and could include emergency departments with admission, medical assessment units, or short-stay wards)
- People who are able to give written consent to take part
Who may not be able to join:
- People who needed a breathing machine (invasive ventilation) during their hospital stay
- People whose doctors expect them to live for less than 90 days
- People whose chest X-ray (if one was taken) showed signs of a new lung infection or consolidation
- People who have been discharged to a residential care home or nursing home after their hospital stay
- People who would not be able to follow or manage a self-care plan with support
- People who do not have access to a telephone
- People who are currently taking part in another interventional clinical trial
- People who have previously taken part in the RAPID trial
