Who might be able to join this trial:
- You are covered by Medicaid or have no health insurance, and you are currently admitted to the hospital
- You are 21 years of age or older
- You have been diagnosed with 2 or more ongoing (long-term) health conditions, OR you are currently taking 2 or more medications for long-term health conditions
- You are currently getting your long-term medications from the hospital pharmacy
- Your long-term health conditions include one or more of the following: diabetes, high blood pressure, high cholesterol, heart disease, heart failure, chronic lung disease, chronic kidney disease, irregular heartbeat, stroke, depression, anxiety, or you are taking blood thinners
Who may not be able to join:
- You are covered by both Medicare and Medicaid at the same time
- The main reason for your current hospital stay is cancer, pregnancy, or surgery for a sudden/emergency problem
- You have been experiencing active psychosis, substance abuse issues, or thoughts of suicide during this hospital stay
- You are not expected to go home when you leave the hospital
- You are already part of a pharmacy program that helps you when you leave the hospital
