Who might be able to join this trial:
- People who are willing and able to give informed consent to take part in the trial.
- People who are willing and able to follow the required procedures before and after surgery in Oxford.
- People aged 21 years or older.
- People who have been diagnosed with central post-stroke pain that has lasted for at least 2 years and has not improved with the best available medical or non-medical treatments.
- People whose usual pain score is greater than 6 out of 10 on a standard pain scale, even after being seen by a specialist pain team.
Who may not be able to join:
- People for whom general anaesthesia (being put fully to sleep for surgery) would be considered unsafe — for example, due to serious heart or blood conditions.
- People who already have a deep brain stimulation (DBS) device implanted in their body.
- People for whom MRI scans are not safe or suitable.
- People for whom brain surgery would not be safe — for example, due to bleeding disorders or an inability to safely stop blood-thinning medication for around 10 days around the time of surgery.
- People with a serious untreated psychiatric or cognitive (thinking and memory) condition that may affect their ability to understand and take part in the trial.
- People with an active skin infection or a type of bacteria that is resistant to most antibiotics (such as MRSA).
- People who are likely to need regular MRI scans after the surgery.
- People who are likely to need certain medical procedures after surgery, such as diathermy (heat-based treatment), ultrasound therapy, or transcranial magnetic stimulation.
- People who are not able to tolerate being awake during surgery.
- People who are not able to manage the recharging of the implanted device.
- People who are pregnant or planning to become pregnant.
- People who, in the opinion of the trial investigator, would not be able to follow the trial requirements.
