Who might be able to join this trial:

• People who have been diagnosed with prostate adenocarcinoma (a specific type of prostate cancer) confirmed by a tissue sample
• Men aged 18 or older
• People whose cancer has returned in a limited way, with no more than 5 spots of disease showing up on a PSMA PET/CT scan, all located in the lower abdomen or pelvic lymph node areas
• People who have at least one area of returned cancer in the pelvic or lower abdominal lymph nodes, with no single tumor larger than 5 cm
• People who previously received radiation treatment to the pelvis for their prostate cancer, and whose cancer responded to that treatment
• People whose cancer is still sensitive to hormones (not yet resistant to hormone-based treatments)
• People who are reasonably physically well, rated 0–2 on a standard medical scale of daily functioning (confirm with trial site)
• People who are able to understand the study and willing to sign consent forms
• People willing to complete quality of life and wellbeing questionnaires
• People willing to be enrolled in the institution's Prostate Cancer Database

Who may not be able to join:

• People without a confirmed tissue-based diagnosis of prostate adenocarcinoma
• People with more than 5 sites of cancer spread in the body
• People who have a history of cancer that has spread to the bones or internal organs
• People with no detectable cancer in the pelvic or lower abdominal lymph nodes
• People who have not had a PSMA PET/CT scan for staging their disease
• People who have previously had radiation therapy outside the pelvis for prostate cancer
• People with a large tumor mass greater than 5 cm in size in the lymph nodes
• People who have received hormone-blocking therapy or chemotherapy within the three months before their staging scan, or at the time of enrolling in the study
• People whose staging scan shows suspicious activity in the prostate itself, unless a prostate biopsy has come back negative
• People with implanted medical devices that would interfere with radiation treatment planning or delivery, as determined by the treating doctor
• People whose cancer has become resistant to hormone treatment (castration-resistant prostate cancer)
• People with a daily functioning score greater than 2 on the standard medical scale
• People with a history of inflammatory bowel disease
• People with a history of any other cancer, except for certain non-melanoma skin cancers
• People who are unable to provide informed consent, or who are currently imprisoned
• People unwilling to complete quality of life and wellbeing questionnaires
• People who do not have the capacity to make their own decisions about participation
