Who might be able to join this trial:

  • Adults aged 18 or older who have been diagnosed with atherosclerotic cardiovascular disease (a condition where arteries are narrowed or blocked by fatty deposits) — confirmed by at least one of the conditions listed below
  • People with a history of a heart attack caused by a blocked artery, or who have had a procedure to open or bypass blocked heart arteries, or who have been found to have significant blockages (more than 50%) in the heart's arteries on a scan
  • People with a history of a serious circulation problem in the legs or arms, or who have had a procedure to restore blood flow to the limbs due to artery disease, or who have had a limb ulcer or amputation caused by artery disease
  • People with a history of having a procedure to treat a blocked neck (carotid) artery, or who have been found to have a blockage of more than 50% in a neck artery on an imaging test
  • People with a history of a stroke caused by a blocked artery (not caused by an irregular heartbeat, heart valve infection, or very low blood pressure), who are currently being treated with low-dose aspirin
  • People who live in Tehran province
  • People who are willing to take part and are able to sign a written consent form

Who may not be able to join:

  • People who have had an unstable or acute cardiovascular event (such as a heart attack, a sudden limb circulation problem, or a stroke) within the past 72 days, or who have had a procedure to open blocked arteries within the past 72 hours
  • People currently taking three blood-thinning medications at the same time
  • People who have had a bleeding episode in the upper digestive tract (such as a stomach bleed) within the past 30 days
  • People who have had a bleed inside the skull or brain within the past 30 days
  • People with very advanced kidney failure (with a kidney filtration rate below 15 mL/min), or who are currently on kidney dialysis
  • People with a serious illness associated with a poor outlook (such as cancer that has spread to other parts of the body) where the treating doctor estimates life expectancy to be less than one year
  • People with other conditions that the trial team considers would make participation or follow-up unsuitable (for example, being unable to read)
  • People for whom aspirin is not planned as an ongoing part of their regular treatment
  • People who are unable to receive or read text messages or phone calls on a personal mobile phone — or whose caregiver living with them is unable to do so on their behalf
  • There may be additional exclusion criteria listed in the full study protocol (confirm with trial site)
