- You have been diagnosed with one of these conditions: Rheumatoid Arthritis, Psoriatic Arthritis, Spondyloarthritis, Ankylosing Spondylitis, Systemic Sclerosis, Systemic Lupus Erythematosus, or Sjogren's Disease
- You have recently been prescribed a new medication to suppress your immune system as part of your treatment for one of these conditions (such as methotrexate, sulfasalazine, leflunomide, mycophenolate mofetil, azathioprine, cyclosporine A, tacrolimus, or one of several types of biologic or targeted therapies including TNF inhibitors, IL-6 inhibitors, IL-17 inhibitors, IL-23 inhibitors, JAK inhibitors, belimumab, or anifrolumab)

- You are being treated with a medication that has not been officially approved for your specific condition
- You are unable to take part in video or phone calls remotely, even with the help of a caregiver
