Who might be able to join this trial:

• People who have been newly diagnosed with stage IV adenocarcinoma (a type of cancer) of the esophagus (food pipe) or the gastroesophageal junction (where the food pipe meets the stomach), with a limited number of cancer spread sites at the time of first diagnosis
• People whose cancer has spread to only a limited number of locations — specifically no more than 2 areas of the body (not counting the original tumor or nearby lymph nodes), no more than 3 tumors within any single organ, and no more than 5 metastases (spread sites) in total
• People whose cancer spread is limited to specific locations, which may include: retroperitoneal lymph nodes (lymph nodes deep in the abdomen), the liver, the lungs, lymph nodes outside the abdomen (such as in the neck or above the collarbone), the adrenal glands (small glands near the kidneys), the ovaries (without spread to the lining of the abdomen), or bones
• People whose treating doctor considers all cancer sites able to be fully treated with local treatments — such as surgery, radiation, or ablation (techniques that destroy tumor tissue) — after an initial course of systemic (whole-body) treatment
• People aged 18 years or older

Who may not be able to join:

• People whose cancer has spread to the peritoneum (the lining inside the abdomen), including those with cancer cells found in abdominal fluid during a diagnostic procedure
• People with a malignant pleural effusion (a build-up of fluid around the lungs caused by cancer)
• People whose cancer has spread to the brain or the membranes surrounding the brain and spinal cord
• People who have cancer spread to any site not specifically listed above that has not been reviewed and approved by the trial's principal investigators (confirm with trial site)
• People who have any cancer site that cannot be fully treated with local therapy
• People who are not well enough to receive the best available systemic (whole-body) cancer treatment
• People whose limited cancer spread developed after an earlier period of having only local disease (called metachronous oligometastatic disease), rather than being present at the time of first diagnosis
• People who have a second separate cancer diagnosis, with the exception of basal cell carcinoma (a common and usually non-spreading skin cancer)
• People who are pregnant, breastfeeding, or planning to become pregnant
• People who are not willing to provide informed consent (formal agreement to participate in the trial)
