Who might be able to join this trial:
For healthcare team members (doctors, nurse practitioners, physician assistants, nurses, and social workers):
- People who work as a doctor or advanced practice provider at a primary care clinic within UNC Health, see at least 60 patients living with dementia per year, and also care for older adult patients — along with their associated nurses and social workers.
- Team members at intervention clinics may also be eligible to receive training with a special advance care planning tool used in this trial.
- For one part of the study (Aim 3), only trained team members from intervention clinics may be eligible to take part in surveys or interviews.
For patients living with dementia (PLwD):
- People aged 65 or older who have been diagnosed with Alzheimer's disease or a related dementia, and who have been seen by a primary care doctor or advanced practice provider at either an intervention or control clinic during the 18-month study period.
- A confirmed diagnosis of Alzheimer's disease or related dementia is required before a patient may be included.
- For one part of the study (Aim 2), patients with a 50% or greater estimated risk of dying within 5 years (from any cause) who are seen during the 18-month study period may be eligible for a healthcare usage analysis.
Who may not be able to join:
For healthcare team members:
- Team members who do not care for older adults (for example, paediatricians or lactation nurses) may not be eligible.
- Team members who work only at geriatric specialty or dementia specialty clinics may not be eligible.
- Team members who do not have a regular primary care patient panel — for example, those working only in urgent care — may not be eligible.
For patients living with dementia:
- Patients who have not had a visit with their primary care team in the past 18 months may not be eligible.
- Patients without a confirmed diagnosis of Alzheimer's disease or a related dementia may not be eligible.
