- People aged 40 years or older
- People who have been diagnosed with COPD, recorded in their medical file, and whose condition falls into GOLD category B or E — these categories are based on symptom scores and how often flare-ups (exacerbations) have occurred, with or without a hospital stay (confirm with trial site if unsure which category applies)

- People who have a diagnosis of dementia recorded in their medical file
