Who might be able to join this trial:

• Men aged 18 or older
• People who have been diagnosed with prostate cancer (a specific type called adenocarcinoma) that requires treatment with radiation therapy
• People whose cancer has been graded as Gleason score 3+3, 3+4, or 4+3 (also called Grade Groups 1, 2, or 3 — meaning low to intermediate risk)
• People whose cancer, as seen on MRI scan, has not spread beyond a certain stage (T3a or lower), and where the MRI was done within the past year
• People who have a tumour visible on MRI scan that meets specific size and imaging criteria (under 2.5cm at its widest point), and where the treating doctor considers it suitable for a targeted radiation boost
• People whose MRI-visible tumour has been confirmed as cancer through biopsy, matching the grade requirements above
• People who are receiving or would normally receive hormone therapy (called ADT) as part of standard care — though hormone therapy is not required if it would not normally be recommended
• People with a PSA blood test result below 20 ng/ml before starting hormone therapy (if hormone therapy is being used)
• People who are reasonably well and able to carry out daily activities (assessed using a standard medical scale called WHO Performance Status 0–2)
• People who are willing and able to understand and sign a consent form
• People who are willing to use contraception during treatment and for one year after, where relevant
• People who are willing and able to complete health and wellbeing questionnaires throughout the study

Who may not be able to join:

• People who cannot have an MRI scan for safety reasons, such as having a pacemaker, certain metal implants in the body, or severe claustrophobia
• People with severe urinary symptoms that the treating doctor considers would make the planned radiation treatment unsuitable
• People with an IPSS score (a urinary symptom score) above 19, indicating significant urinary problems
• People who have high-grade cancer (Grade Group 3) in areas of the prostate that do not show up on the MRI scan
• People whose prostate gland is larger than 90cc in volume
• People with other health conditions that may increase the risk of serious side effects from treatment or prevent long-term follow-up (for example, inflammatory bowel disease)
• People who have a hip replacement or other metal hardware in the pelvis that significantly interferes with MRI scan image quality
• People who have previously had radiation therapy to the pelvic area
• People whose disease means they would need more than 6 months of hormone therapy
• People who have had another cancer (other than certain skin cancers, low-risk bladder cancer with a recent clear check, or small kidney growths being monitored) within the past 2 years that is likely to affect life expectancy
• People who are already taking part in another interventional clinical trial for prostate cancer
