Who might be able to join this trial:
- Adults between 18 and 80 years old who have had an acute ischemic stroke affecting the front part of the brain (anterior circulation).
- Brain imaging shows the stroke was caused by a blockage in a specific artery (the end section of the internal carotid artery, or the M1/M2 section of the middle cerebral artery).
- A brain CT scan taken within 24 hours of stroke onset shows an ASPECT score of 6 or higher, and the person meets standard medical guidelines for a clot-removal procedure called thrombectomy.
- Before the stroke, the person had little to no disability (a pre-stroke mRS score of 1 or less).
- The stroke severity score (NIHSS) before the clot-removal procedure is between 6 and 25.
- The clot-removal procedure achieved successful restoration of blood flow (mTICI score greater than 2b or 3) (confirm with trial site).
- The person, or their representative, has signed a consent form agreeing to participate.
Who may not be able to join:
- People for whom TMS (a type of brain stimulation treatment) is not safe, such as those who have metal objects in their head, a pacemaker, an implantable drug pump, or a cochlear implant.
- People with epilepsy, high pressure in the brain, brain tumours, acute brain bleeding, other serious neurological conditions, or severe problems with the heart, liver, or kidneys.
- People whose brain CT or MRI scan shows a dangerous shift of brain structures to one side, a significant mass effect, or who are being considered for brain surgery.
- People whose brain CT or MRI scan shows acute stroke damage on both sides of the brain.
- People who are pregnant or breastfeeding.
- People with severe mental health conditions or dementia that would prevent them from taking part in follow-up assessments.
- People with other serious illnesses where survival beyond 90 days is not expected.
- People who are currently taking part in, or have taken part in, another interventional clinical study within 30 days before this study begins or since the stroke occurred.
- People who are unable to give or agree to informed consent.
