Who might be able to join this trial:

• People who have a narrowing of 50% or more in the carotid artery (a major blood vessel in the neck), confirmed by an ultrasound, MRI scan, CT scan, or catheter imaging.
• People who have had a mini-stroke (TIA) or a mild stroke with low symptom severity, on the same side as the narrowed artery, within the last 24 hours.
• People who are conscious, neurologically stable, and either have no signs of brain damage from reduced blood flow, or have a small area of brain affected that measures less than 25mm across.
• People aged between 45 and 90 years old.
• People with an ASA health score below 4 (a measure used by doctors to rate overall physical health before a procedure).
• People who are able and willing to attend follow-up appointments as required by the trial.
• People who are willing to give their informed consent to take part in the study (with a special process in place for people who are temporarily unable to consent due to speech difficulties, as assessed and documented by their treating doctors).
• People who received clot-busting treatment or a clot-removal procedure after their stroke or TIA, provided a subsequent brain scan showed no bleeding in the brain.

Who may not be able to join:

• People whose carotid artery narrowing is less than 50%, as confirmed by imaging.
• People who have a blood clot blocking the carotid artery or a tear in the artery wall.
• People whose stroke symptoms are rated as more severe (above a score of 5 on the NIHSS medical scale).
• People who have bleeding in the brain.
• People who are unconscious or whose neurological condition is changing or unstable.
• People with cancer, a serious heart condition, a severe neurological disorder, or any other condition associated with a poor outlook.
• People whose brain scan shows an affected area larger than 25mm in diameter.
• People whose brain scan shows lesions of uncertain or unknown cause.
• People who have had repeated mini-strokes or whose stroke symptoms are still getting worse.
• People younger than 45 or older than 90 years of age.
• People with an ASA health risk score of 4.
• People who are unable or unwilling to give informed consent, where the special consent process does not apply.
• People for whom surgery on the carotid artery cannot be performed within 72 hours of the first stroke or TIA symptoms.
• People who are unable to attend a follow-up appointment 90 days after their initial symptoms.
• People who have previously had surgery or a stent procedure on the carotid artery being studied.
