Who might be able to join this trial:
- You are an outpatient (not currently admitted to hospital), of any sex, and are 65 years old or older
- You have been diagnosed with atrial fibrillation (an irregular heartbeat) — whether newly diagnosed or long-standing — confirmed by a heart tracing test (ECG) or a monitor worn over a period of time
- You have at least one other long-term health condition alongside your atrial fibrillation, such as: high blood pressure (treated with at least 2 blood pressure medications), coronary artery disease, peripheral artery disease, heart failure, stroke or mini-stroke (TIA), diabetes, chronic lung disease (COPD), or chronic kidney disease (CKD)
Who may not be able to join:
- You have an artificial (mechanical) heart valve or a significant narrowing of the mitral valve in your heart
- You are not willing to take part or sign a consent form agreeing to join the study
- You are unable to understand what the study involves or cannot attend follow-up appointments
- You have a serious illness where life expectancy is expected to be less than 12 months
- You are already taking part in another clinical intervention study
- You do not have a compatible device for use with the study's digital tool called iABC (this applies only to certain trial locations) (confirm with trial site)
