Who might be able to join this trial:
- People aged 18 or older with a life expectancy of more than 15 years
- People with prostate cancer confirmed by biopsy, classified as WHO/ISUP grade group III, IV, or V
- People with high-risk prostate cancer, which includes either a biopsy-confirmed WHO/ISUP grade group III–V, or grade group II with a PSA level above 20 ng/ml
- People whose PSMA-PET scan shows no signs of cancer spread to nearby lymph nodes or distant parts of the body
- People for whom a specialist medical team has recommended surgical removal of the prostate (radical prostatectomy)
- People in good general health, rated as WHO performance status 0 or 1 (meaning fully active or able to carry out light activity)
- People whose overall health is suitable for general anaesthesia and major surgery (rated ASA III or lower) (confirm with trial site)
Who may not be able to join:
- People with the lowest-risk prostate cancer classification (ISUP grade group I) and early-stage disease on MRI scan (cT1 or cT2)
- People whose MRI scan shows the cancer has grown beyond the prostate into nearby structures (cT4 stage)
- People whose PSMA-PET scan shows cancer has spread to nearby or distant areas of the body
- People who have already received any treatment for prostate cancer, including hormone therapy, radiation, or other systemic or local treatments
- People who have previously had lymph node removal in the pelvic area or radiation treatment to the pelvis
- People who have had another type of cancer in the past, unless that cancer was fully treated at least 2 years before joining the trial and there is no current sign of that cancer returning (a shorter time may be acceptable for cancers that are unlikely to come back — confirm with trial site)
- People with serious medical, mental health, psychological, family, or practical circumstances that, in the opinion of the medical team, could interfere with treatment, follow-up, or safety
- People who are not able to make their own informed decisions, or who are under legal guardianship
