Who might be able to join this trial:

For patients:

• You are 18 years of age or older
• You have been diagnosed with breast cancer that has spread to other parts of the body (metastatic breast cancer)
• You are within 8 weeks of a clinic visit where a specific computer-based health score suggested you may benefit from end-of-life care planning (confirm with trial site)
• You are able to carry out daily activities with minimal or moderate limitations, as assessed by your cancer care team
• You receive your cancer treatment at one of the hospitals or clinics taking part in this study
• You are able to fill out questionnaires in English or Spanish

For caregivers (family members or friends):

• You are 18 years of age or older
• The patient has identified you as a family member or close friend who is mainly involved in helping with their care
• You are able to fill out questionnaires in English or Spanish

Who may not be able to join:

For patients:

• You have had an outpatient palliative care visit (specialist comfort or supportive care) in the last six months
• You are currently enrolled in hospice care
• You have another health condition that affects your ability to understand the study or give your consent to take part, according to your cancer doctor
