For patients:
- You must be able to speak English
- Your planned discharge destination should be home, assisted living, or long-term care

- The facility must admit at least 75 skilled nursing patients per year
Who may not be able to join:
For patients:
- You may not be eligible if you already have a planned return to hospital for a procedure or treatment within 30 days of joining the trial
For caregivers:
- You may not be eligible if your role as legal representative was assigned by a court (for example, a court-appointed guardian)
